Become an A20labs Patient

If you need a Colorado Medical Marijuana License:

Simply call 877-536-2828 or email Care@a20labs.com to understand if you could be a canidate for obtaining a Colorado MMJ License and to arrange your first consultation with an A20labs patient manager. A20labs will pay for your Colorado State medical marijuana license application fee ($90).

To prepare for your first meeting please complete the New Patient Registration Form/History. Print the document, fill it out, and hang onto it for your first meeting with A20labs. Also make a copy of your Co Driver License.

If you have a Colorado Medical Marijuana License:

1.) Print the 'New Patient Registration Form/History' form using the link on the right hand side of this page. Fill out the form and hang onto it for your first visit with an A20labs patient manager. This form will help A20labs build an effective treatment program for you.

2.) Print the 'Primary Caregiver Agreement' form using the link on the right hand side of this page. Fill out the form and hang onto it for your first visit with an A20labs patient manager. As you fill out the form please indicate in the space provided on the form if you would like to become a patient of A20labs for 45 days, 3 months, 6 months, or 1 year. The choice is yours. 3.) Print the 'CO State Change of Caregiver' form using the link on the right hand side of this page. Fill out the form and hang onto it for your first consultation with an A20labs patient manager.

4.) Make clear photocopies of your drivers license, the CO State MMJ "PHYSICIAN CERTIFICATION" form used to obtain your MMJ license, and the CO State MMJ "APPLICATION FOR IDENTIFICATION CARD" form used to obtain your license. Hang onto these for your first visit with an A20labs patient manager.

5.) Go to the 'Products' page on this website and identify any products that are immediately needed for your treatment.

6.) Call 877-536-2828 or email Care@a20labs.com to arrange a visit with an A20labs patient manager. Indicate to the representative any products that you may immediately require. At this first consult a long term treatment program will be discused.
